Since this is your first time coming to Orthopedic Associates of Dayton, please review and fill out the forms listed below. This will help expedite your check-in and possibly shorten your wait time. Additionally, please remember to arrive 15 minutes before your scheduled appointment time to answer any additional questions. Please remember to bring your insurance card for every visit and photo ID. Patients who have not been seen for one year will be required to complete another new patient form. Our providers see patients Monday through Friday at our Dayton location. Since each provider has a different schedule, patients are seen by appointment only.
We receive telephone calls from 8:00am-4:30pm, Monday through Friday. When our office is closed the Medical Society answers our phones and they will contact the physician on-call for you. In the event of a medical emergency, you should call 911.
If you need a refill on a current prescription, you may call (937) 280-4988 and select the prompt for your providers Medical Assistant. You can also send a refill request to your provider through our patient portal.
Messages may be left any time of day. Please call well in advance as it may take up to 48 hours to complete your request.
If you feel you need to change a medication, or have other questions about your prescription, please call during normal office hours
